Introduction: Longitudinal extensive transverse myelitis (LETM) is defined as inflammation spinal cord lesion that extends over three or more vertebrae. The purpose of this case report is to present PT examination and management strategies for a single patient with LETM. Case Description: A 25-year-old male patient diagnosed with LETM referred to PT. He presented foot drop, ataxic gait, decreased sensation, bilateral lower extremities weakness, significant fatigue, and low back pain. The patient required significant rest time between all tests and measures due to severely increased fatigue. PT plan of care was focused on therapeutic exercises per patient exercise tolerance. Methodology: Therapeutic protocol which administered weekly six 6 days for 24sessions. Therapeutic treatment protocols consist of AROM, Strengthening PRE, FRT Gait corrected Coordination and balance exercise which was carried out 1-hour duration of treatment. Prior treatment warms up 15 minutes followed by 45 minutes exercise session. Results: Pre and post outcome score 4(pevaluated through FMS. Conclusion: The therapeutic treatment activities based on patient exercise tolerance is important when treating people with LETM. Through this approach which help to improve mobility, gait, coordination, balance, independent, socially acceptable, Quality and livelihood of the patient. Early diagnosis helps to reduce morbidity of LETM. Key Words: LETM, FMS, PRE and FRT.
